Initial Location Assessment Form

The Initial Location Assessment only needs to be completed once and returned to Milwaukee Riverkeeper in person, by mail, or by scanning and emailing to adoptariver@milwaukeeriverkeeper.org. If anything indicated on the form drastically changes over time, please complete and return another Initial Location Assessment.

Cleanup Report Form

Please fill out a Cleanup Report Form for each of your cleanups and return it to Milwaukee Riverkeeper in person, by mail, or by scanning and emailing the form to adoptariver@milwaukeeriverkeeper.org within one week of your cleanup.

Co-Branding Agreement

Full-Length Liability Waiver

All volunteers must read and sign a liability waiver before participating in a river cleanup. Volunteers under the age of 18 must have a parent or guardian sign a waiver on their behalf. Milwaukee Riverkeeper’s Short-Form Liability Waivers (below) help save paper in large volunteer groups!

Todos los voluntarios deben leer y firmar una exención de responsabilidad antes de participar en la limpieza de un río. Los voluntarios menores de 18 años deben tener un padre o tutor que firme una renuncia en su nombre. Las exenciones de responsabilidad de formato corto de Milwaukee Riverkeeper (abajo) ayudan a ahorrar papel en grandes grupos de voluntarios.

Short-Form Liability Waiver

These sign-in sheets save paper and may be used in place of the full-sheet waiver as long as all volunteers still read the full-length waiver.
